New Patient Intake

You may skip the section regarding the request for the insurance card. We will take care of that in the office. Please make sure to bring your insurance card with you so that we may check your benefits.

If you are unable to complete the intake form you will need to arrive 30 minutes before your appointment time to fill out the form.

​Although unusual for a doctor’s office, we strive to have all patient appointments begin right on time, at your scheduled time. We agree that your time is just as valuable as ours. Please keep this in mind when scheduling your drive time and arrival time.
